Parent Version of the Young Mania Rating Scale (P-YMRS)

The P-YMRS is a brief questionnaire completed by a parent or caregiver to identify and quantify the intensity of manic symptoms in children and adolescents. It notably explores elevated or irritable mood, increased energy, decreased sleep, accelerated speech and thinking, unusual thought content, aggressiveness, appearance and insight. In clinical practice, it is mainly used as a screening tool and for monitoring symptom changes when pediatric bipolar disorder or mood instability is suspected. It is quick to administer, which makes it useful in outpatient practice, child psychiatry and research. It cannot establish a diagnosis on its own and must always be interpreted together with the clinical interview, the developmental history and contextual information.

Scoring & interpretation

The questionnaire comprises 11 items. Items 1, 2, 3, 4, 7, 10 and 11 are rated from 0 to 4. Items 5, 6, 8 and 9 are weighted and rated from 0 to 8. The total score is obtained by summing the highest score retained for each item, for a total ranging from 0 to 60. The higher the score, the more significant the manic symptomatology reported by the parent. In screening, a score of around 18 or more is a warning sign warranting an in-depth diagnostic assessment, without being sufficient to confirm bipolar disorder.
