Facial Emotions Recognition Test (TREF)

The TREF is a social cognition test designed to assess the ability to recognize facial emotional expressions in adults. It is based on 54 color photographs of male and female faces expressing six basic emotions at nine intensity levels, which makes it possible to estimate both the accuracy of identification and the sensitivity to low-intensity expressions. The tool is particularly relevant in neuropsychology and adult psychiatry, especially for identifying the emotion recognition difficulties observed in schizophrenia and other clinical presentations involving impaired social cognition. Its clinical value lies in the analysis of the overall score, performance by emotion, specific confusions and the detection threshold, in order to refine the interpretation of the assessment and guide targeted remediation.

Scoring & interpretation

Each photograph requires choosing one emotion among six options. Scoring is based on the number of correct responses, with a total score out of 54, generally converted into a percentage, and a subscore per emotion out of 9. The test also makes it possible to estimate a detection threshold for each emotion based on the minimum intensity correctly recognized or, according to some clinical procedures, based on the first correct recognition and the level from which recognition becomes stable. A high score indicates better emotion recognition. In the original study, an overall score below 61.57% suggested a probable deficit, a cutoff to be interpreted with caution given the reference population and the clinical context.
