WFIRS-P
Weiss Functional Impairment Rating Scale-Parent Report (WFIRS-P)
The WFIRS-P is an observer-rated questionnaire completed by a parent or caregiver to assess the functional impact of attention-deficit/hyperactivity disorder, or more broadly of emotional and behavioral difficulties, on the daily life of the child or adolescent. It explores the concrete repercussions on family life, school, life skills, self-concept, social relationships, and risky behaviors. The tool is particularly useful in clinical assessment and treatment monitoring, and for objectively documenting functional impairment as distinct from symptoms alone. It helps identify the most affected domains and track their changes over time.

Scoring & interpretation
Each item is rated from 0 to 3 according to the intensity of the impact observed over the past month, with a "not applicable" option. Items marked not applicable are excluded from the calculation. Preferably, a mean is calculated for each domain to identify the most affected areas. In clinical practice, a domain can be considered impaired if at least two items are rated 2, if one item is rated 3, or if the domain mean exceeds 1.5. The total score can be summarized as an overall mean, but interpretation by domain remains the most clinically useful.
